1. Introduction to HFACS is intended to be a free online course designed to provide the participant with an overview of the HFACS system. The audience is assumed to be safety professionals exploring options for implementing an accident investigation and analysis system in their workplace.

Course Title: Introduction to HFACS
Medium: CBT
Course length: Approx 15 minutes
Cost per participant: Free
Course Objectives: At the end of this online course the participant will be able to:

Motivate participant and potential clients to pursue active use of the HFACS system

2. HFACS Accident Investigation is intended to provide the student with the skills and knowledge necessary to conduct accident investigations and take immediate actions to reduce risk.

Course Title: HFACS Accident Investigation
Medium: CBT
Course length: Approx 45 minutes
Cost per student: $199.00
Course Objectives: At the end of this online course the participant will be able to:

Given an Accident investigation scenario, conduct an accident investigation using the tools provided and recommends immediate actions to reduce risk.

Enabling Objectives:
  • Define terms associated with Accident Investigation.
  • Identify the purpose of an Accident Investigation
  • Describe the Accident Investigation process.
  • Discuss the HFACS system
  • Identify common causes of accidents.
  • List and describe the key steps in the Accident Investigation process.

3. Basic HFACS is designed to be an entry level or review course for interested practitioners involved in the accident investigation/analysis process. It is assumed that these are safety practitioners with some experience as accident investigators. It does not delve deeply into analysis and intervention; rather, it sets the participant up for the “HFACS Analyst course” to be developed at a later date. We just want the participant at this stage to correctly identify which level and subcategory of the Taxonomy the causal factors may reside.
Course Title “Basic HFACS”
Medium: CBT (multiple modules)
Course Length: 2-3 Hours based upon speed of participant (self paced)
Cost per student: $299.00
Course Objectives:

Terminal learning Objective: At the end of this online course the participant will be able to:
Given a complete accident/incident report and a basic HFACS Taxonomy, correctly identify the relevant human error level and causal categories in accordance with the instruction provided.

Module 1:

  1. Discuss the history of HFACS
  2. Describe the 6 human error perspectives
  3. Identify the theorist that achieved the most significant success in bringing these 6 perspectives together

Module 2:

  1. Describe Reason’s accident causation model
  2. Discuss the Strengths and Weaknesses of the “Swiss Cheese” Model
  3. State the reasons for development of the HFACS system

Module 3:

  1. Define the following levels within the HFACS system:
    • Unsafe Acts
    • Preconditions for Unsafe Acts

4. HFACs Analyst is designed for provide basic HFACS users with the skills and knowledge necessary to accurately identify causal factors and implement applicable and effective interventions to reduce risk.

Course Title “HFACS Analyst”
Medium: CBT
Course Length: 2 Hours based upon speed of participant (self paced)
Cost per student: $299.00
Course Objectives:

Terminal learning Objective: At the end of this online course the participant will be able to:
Given a complete accident/incident report and identification of specific levels within the HFACS system containing potential causal factors, accurately determine causal factors using basic nano-codes and determine and implement solutions to reduce risk.

  • Review the HFACS system

5. HFACS Proactive
is designed to provide the experienced HFCS user with the skills and knowledge necessary to use the HFACS system in a proactive manner, reducing risk in the workplace.

Course Length: 1 Hours based upon speed of participant (self paced)

Course Objectives:

Given a medium to large company based scenario, leading and lagging indicators and knowledge of the HFACS system, take proactive measures to reduce risk in the workplace.

  • Describe workplace systems and processes.
